What do I need to sign a declaration with my family doctor?
The patient comes to the appointment with his/her passport, ID code and mobile phone (it is no longer necessary to make copies of documents). The doctor or nurse enters the data into the electronic database. If the person does not live where he or she is registered, the actual address is also indicated. A password is sent to the mobile phone, which is used by the health care provider to log into the electronic system and enter the patient's data. The declaration is then printed out in two copies, which are signed by the doctor and the patient. One copy remains at the health centre and the other with the patient.
A person can sign the declaration on behalf of other family members - a child or an elderly relative who does not use a mobile phone. Parents or guardians only need to come with their documents, bringing the child's birth certificate or the elderly person's passport and code.
Signing the declaration guarantees that, in addition to consultation with a specialist, a person will be able to take general blood and urine tests, check blood glucose and cholesterol levels free of charge. One will not have to pay for cardiogram and fluorography. It will also be possible to undergo tests for HIV infection and viral hepatitis.
