Types of Joint Replacement
There are three types of artificial joints worldwide, differing in the material of their friction surfaces:
- Metal-polymer pair: The most common option, where one part is metal and the other is made of high-density polyethylene. It is relatively affordable but wears down over time.
- Ceramic implants: These allow MRI scanning, wear out minimally, but cannot withstand heavy loads and are more expensive than other options.
- Metal-on-metal implants: These provide maximum durability, making them ideal for high-load joints. They are non-magnetic and do not interfere with MRI scans or set off metal detectors.
Another important aspect of arthroplasty is the extent of joint surface replacement, which can be total or partial:
- Total joint replacement: The entire joint is replaced, including all articulating surfaces (common for hip replacements).
- Partial joint replacement: Only one joint surface is replaced (often performed on the knee, such as the patella or part of the femur).
Fixation Methods in Joint Replacement Surgery
There are two types of fixation methods used in arthroplasty:
- Cemented fixation: The artificial joint is secured using a strong polymer adhesive (bone cement). This is ideal for older patients with low bone density or osteoporosis.
- Cementless (press-fit) fixation: The implant's surface is coated with a material that stimulates bone growth, allowing the bone to naturally integrate with the prosthesis. This method is suitable for younger, physically active individuals (up to 40–50 years old).
Types of Arthroplasty by Purpose
- Primary arthroplasty: The replacement of a damaged joint with an artificial prosthesis.
- Revision arthroplasty: Surgery performed on a previously implanted prosthesis to repair or replace worn-out parts.
The choice of arthroplasty type, implant material, and fixation method is determined individually by the doctor, considering the patient's needs, preferences, and financial options. A properly performed joint replacement can provide 15–20 years of pain-free function, as modern prostheses are highly durable.
